The Failure Modes and Effects Analysis (FMEA) Tool is a method for analyzing any process to see where it is likely to fail; to see how changes you are considering might affect the safety of the process; and to track changes in the Risk Priority Number (or RPN) of the process over time.




The Trigger Tool for Measuring Adverse Drug Events (ADEs) is a simple yet powerful method of identifying and tracking the number of medication errors in your hospital — using a retrospective review of patient records to look for sentinel clues or "triggers" that indicate a medication error has occurred.
