Forms are a means by which information may be gathered and reported on. Forms are made up of a series of questions with the ability for the user to enter their response.
Forms are created by clicking
on the Forms link on the Administrators tab.
This displays the Manage Forms page, there are three types of forms:
Deliverable Data Form - this is used
to capture information during the project e.g. reports on progress, this
is covered in more detail in the Deliverable
Assignments section.
Visit Data Form - this is used to
capture non aggregated data for example, capturing information about a
single patient visit.
Team Profile - this is used to capture
additional information on each team and would be completed ONCE
by, for example, the Key Contact (it is possible to have more than one
form but they will be displayed in sequence as a single form in sections).
To share the form across all projects, saving duplication, click on the Share link adjacent to the form to be shared.
Click on one of the form types at the top of the Manage Forms page (in this example the Add Visit Data Form option is used.
The Configure Form page is displayed, it is split into two sections:
The Internal name and description - these details will appear in the Forms list.
The Display
name and description - these details will appear on the displayed forms.
Complete the Internal details.
Complete the Display
details.
Click the 4SAVE button to save the form information.
