Forms

Forms are a means by which information may be gathered and reported on. Forms are made up of a series of questions with the ability for the user to enter their response.

Forms are created by clicking on the Forms link on the Administrators tab.

This displays the Manage Forms page, there are three types of forms:

To share the form across all projects, saving duplication, click on the Share link adjacent to the form to be shared.

Creating a Form

  1. Click on one of the form types at the top of the Manage Forms page (in this example the Add Visit Data Form option is used.

  2. The Configure Form page is displayed, it is split into two sections:

  3. Complete the Internal details.

  4. Complete the Display details.

  5. Click the 4SAVE button to save the form information.
